What we collect
Information you give us
When you request an appointment through this site, you choose what to share: who the visit is for, the reason, an optional note, your preferred office and time, your name, phone number, email address, and insurance selection. The site also records the time of the request, a shortened cryptographic fingerprint of your network address (used only to limit abusive automated submissions), and your browser type.
Searching the site sends your search words to our server to find matching pages. Search queries are processed to return results and are not stored with your identity.
How it is used
Scheduling, and nothing else
Appointment request information is used to contact you and arrange your visit: it is delivered to the practice’s office staff and referenced by the confirmation number you are shown. It is not used for marketing, not shared with advertisers, and not sold. Ever.
Where it lives
Storage and security
This website runs on Cloudflare’s infrastructure. Appointment requests are stored in a database dedicated to this practice, reachable only by the practice and the people who operate this site for it. Transport is encrypted (HTTPS) everywhere. Cloudflare, as the host, processes network logs to serve and secure the site.

- Cloudflare Turnstile
- Bot protection on form submissions, when enabled
No advertising networks, tracking pixels, or data brokers are used on this site.
Medical records
This website is not your medical record
This website is a scheduling and information surface, not a medical record system, and nothing you submit here becomes part of your chart until the office enters it. The practice’s Notice of Privacy Practices, which governs your health information under federal law, is available from either office. For records requests, call the office that holds your chart.
Children
Requests for children come from adults
This site is not directed at children. Appointment requests for a child are made by a parent or guardian, and the information is handled exactly as described above.

To review, correct, or ask us to delete information you submitted through this site, call either office and reference your confirmation number if you have it. See also Your Privacy Choices and the cookie policy.
